• ARTISAN training:
  The registration fee for ARTISAN is US $ 2500.
  This includes: seminar fee, wetlab, ARTISAN instrument set, hotel, dinner and lunch.
  The Instrument kit for ARTISAN contains: 
  - Implantation Forceps Standard 
  - Implantation Forceps Refractive, Long 
  - Enclavation Forceps 
  - Lens Manipulator Standard, Straight 
  - 1 box enclavation needles
  - 5 Aphakic ARTISAN lenses
   
• ARTISAN and ARTIFLEX training:
  The registration fee for ARTISAN / ARTIFLEX is US $ 3000.
  This includes: seminar fee, wetlab, ARTISAN and ARTIFLEX  instrument set, hotel, dinner and lunch.
  The Instrument kit for ARTIFLEX contains:
  -Implantation Forceps Left
  -Implantation Forceps Right
  -Lens Manipulator
  -1 box enclavation needles
  -1 box disposable spatulas

• Registration is, on first-come-first-served base, limited to 20 persons maximum.
• Forms received with no proof of payment will not be considered.
